A 11 month old child arrives at the clinic with a 31 hour history of a somewhat of a sore ear. The father reports no fever. The child stays at home with the mother and has an older sister with a cold. The child is not immunized.

Physical Exam: Child is in pain. Ear: See picture. Remainder of physical exam is unremarkable.
