A male patient, age 78, has come in for an annual screening. 

History: Patient is sedentary, with less than 30 minutes of physical activity daily. Does not eat fruits or vegetables daily. Brother has diabetes. Has been on a blood pressure medication for the past 5 years. Had a high glucose when tested 10 years ago.

Physical Exam: Patient has a bmi of 38 with a waist circumference of 113 cm.
