A 11 month old child arrives at the clinic with a 60 hour history of a marginally sore ear. The mother reports no fever. The child attends swimming lessons and has an older sister with a cold. The child is not immunized.

Physical Exam: Child is uncomfortable. Ear: See picture. Remainder of physical exam is unremarkable.
