In the 0-1 month population, a low threshold for suspecting sepsis must be had. Any history of poor feeding (without obvious minor cause), lethargy, low or high temperatures all prompt immediate full septic workup and treatment in the hospital. Risk factors for sepsis include untreated GBS status, maternal fever during delivery, active vaginal lesions during delivery. Even though babies may examine without a source of infection, the temperature in and of itself make outpatient treatment, workup or reassurance inappropriate. In babies without fever who examine well and are hungry but have trouble feeding after a few minutes, you may try suctioning nasal congestion and re feeding.
